After the EMR analysis is conducted a list of patients who is eligible to qualify for the CCM program is made by the specialists at Frontizo and then they present it to the Practice.

3. Billing Orientation & Training

Every billing staff at Frontizo has been specially trained on how to get reimbursed for the Chronic Care Management services in the right way and offer clients with all the required documents. Our aim is to make the implementation process for our clients a hassle-free affair.

4. Staff Training

Our staffs have been thoroughly trained on the ways in which they should introduce patients about the CCM program and also at the same time making them aware of the advantages of how our practice cares for the chronologically ill patients.

1. PATIENT CONSENTED TO PROGRAM

2. CARE MANAGER REVIEW

The Care Managers at Frontizo on behalf of the Practice will reach out to the patients, explain them about the CCM program and declare everything that is mandated by CMS. The care managers obtain verbal consent and document the decision of the patient in the EMR. They will review the chart PCP notes of every patient and get in touch with their Specialist for verification.

3. CCM PROGRAM INITIATION

Finally, the CCM program will be initiated and the care manager will henceforth keep a track of all the communication and relevant patient information in the Frontizo Portal. They are experts in documentation and tracking needed to fulfill CMS requirements.

4. EMR UPDATE & DOCUMENTATION

After all this is done, the care manager will input in the Client’s EMR System all the necessary and relevant CCM notes.
